Because WSU Health is a separate legal entity from Range Community Clinic, WSU Health will not have access to your existing medical records unless you complete and return a signed Release of Information form. This form is included with this letter and must be returned if you want WSU Health and your provider to receive your medical records to continue your care.
You may access your medical records in one of the following ways:
- By calling Range Community Clinic before January 30, 2026, at 509-505-7481 Option 1
- After January 31, 2026, by contacting Morgan Records Management at:
We encourage you to establish care with WSU Health or another health care provider as soon as possible to ensure continuity of care. If you want your medical records transferred to WSU Health or another provider in the community, please complete the enclosed Release of Information form and return it to us in the enclosed self-addressed envelope or, after January 31, 2026, to Morgan Records Management.
